PRE-REGISTRATION

Stockholders as of record date (April 1, 2024) who would like to attend the virtual meeting are requested to pre-register no later than April 26, 2024 by sending all the applicable requirements below to ASM2024@gtcapital.com.ph. Submission of incomplete requirements may result in unsuccessful event registration.

For Certificated Stockholders:

Individual Stockholders

• A scanned copy of the Stockholder's valid government-issued ID showing photo, signature and personal details, preferably with residential address. The file size should be no larger than 2MB;
• A valid and active e-mail address and contact number; and 
• Accomplished Data Privacy Consent Form, downloadable here

Corporate Stockholders

• A secretary’s certificate attesting to the authority of the representative to participate by remote communication for, and on behalf of the Corporate Stockholder. The file size should be no larger than 2MB;
• A scanned copy of the valid government-issued ID of Stockholder’s representative showing photo, signature and personal details, preferably with residential address. The file size should be no larger than 2MB;
• A valid and active e-mail address and contact number; and
• Accomplished Data Privacy Consent Form, downloadable here

For Stockholders under PCD Participant/Brokers account or “Scripless Shares”:

• A broker certification on the Stockholder’s number of shareholdings. The file size should be no larger than 2MB;
• A scanned copy of the Stockholder’s valid government-issued ID showing photo, signature and personal details, preferably with residential address. The file size should be no larger than 2MB;
• A valid and active e-mail address and contact number; and
• Accomplished Data Privacy Consent Form, downloadable here
